For appointments with Dr. Aydin, failure to cancel your appointment at least one full business day before your scheduled time will result in a fee of $100. This also applies to No-Shows for Dr. Aydin’s appointments. Business days include Mondays through Fridays. Monday appointments must be cancelled by Friday. As for all other appointments with other providers and staff, the fee for failure to cancel at least one full business day before your scheduled time will result in a fee of $50. This also applies to No-Shows. A credit card will be taken at the time of your appointment and will be charged the aforementioned fee in case of failure to cancel within the prescribed period.
You must present government issued identification with you at the time of your service. Please understand that this is a medical office and we will need this information as part of your patient chart. In the event you do not present such identification, your appointment will be cancelled.
